Health insurance is one of the most important topics for anyone moving to Mallorca. Spain has a good public health system, but not everyone qualifies automatically. Here are your options.

Public health insurance (Seguridad Social)

As an employee or Autónomo in Spain, you are automatically covered by the Seguridad Social:

  • GP visits at your local Centro de Salud
  • Specialist treatment (with referral)
  • Hospital stays
  • Medication (with co-payment based on income)

Cost: Included in social security contributions. As Autónomo from approx. 230 euros/month (reduced rate for new businesses: from 80 euros).

Convenio Especial – buying into the public system

Retirees and non-working residents can join the Seguridad Social via Convenio Especial:

  • Under 65: approx. 60 euros/month
  • Over 65: approx. 157 euros/month

Requirements: Empadronamiento (municipal registration) and at least 1 year in Spain or previous membership.

Private health insurance

For many expats, private health insurance is the best option, especially initially:

  • Immediate coverage from day 1
  • Free choice of doctor, short waiting times
  • English-speaking doctors and clinics
  • Accepted as proof for residency applications

Cost: From approx. 50-100 euros/month. Popular providers: Sanitas, Adeslas, DKV, Asisa.

Using your home country insurance in Mallorca

Public insurance from your home country: Generally does not apply after emigrating (exception: posted workers with an A1 certificate). The EHIC (European Health Insurance Card) only covers temporary stays.

Private insurance: May potentially continue coverage abroad. Check the conditions for long-term stays and whether Spanish healthcare providers can bill directly.

Recommendation

Most expats start with a Spanish private health insurance for their residency application and switch to the public system once they are employed or join via Convenio Especial.
